Advanced Skilled Home Health Care
Avocare Home Health Services provides skilled home health services integrating virtual and in-person care through a 24-hour clinical call center with the availability of Remote Physiological monitoring and telehealth services. Our clinical staff and leadership are experienced and trained in Dementia, Alzheimer's Disease and Memory Care as well as person-centered care that respects Self-Determination. We ensure effective Transitions of Care to avoid unnecessary readmissions through Admission, Discharge and Transfer monitoring for ED and Inpatient admissions. Avocare ensures that all patients are screened for eligibility and are referred for all services they need including community and governmental agencies and organizations. Avocare works with all community and governmental organizations and assists patients and families with applying for support services and resources.
Skilled Home Care Services
We provide all skilled home care services including RN, PT, OT, ST, LMSW, & CHHA. We also have Behavioral Health Services provided by Licensed Therapists and Psychiatrists.
24 Hour Nurse Call Center
Our RN clinical call center is available 24-hours daily 365 days a year using the latest technology and evidenced-based triage tools to intervene accurately and safely for patients.
ADT Alerts & RPM
We monitor Admissions & Discharge Alerts for Transitions of Care to prevent readmissions. We provide remote physiological monitoring for patients when indicated.
Behavioral Health
We coordinate & close care gaps for behavioral patients, families & providers. We are experienced in providing Dementia and Alzheimer's care with the presence of behaviors and SMI. We deliver collaborative Behavioral Health Integration (BHI) with Licensed Therapists & Psychiatrists.
Personal Care and Homemakers
We provide experienced, trained and caring personal care and homemaking staff that are trained in Dementia and Alzheimer's care as well as Memory Care. We ensure our staff use a person-centered approach to care through ongoing training.
