We created Avocare Health Services to keep patients connected to their providers, resources and loved ones in their communities where they want to live. From Skilled Home Care delivered in the home to Virtual Care delivered through our call center, we actively engage patients to keep them informed and assess changes in their health or living situation. We leave no patient behind and can keep them connected throughout their life. We believe all patients should be able to age in place in their own home with dignity, independence and freedom. We do all things necessary to make that possble.
Our Community Our Focus
Avocare Health services is an organization that is owned and operated by minority women whose singular focus is to ensure that the most underserved communities achieve health equity and health care equity through dedication to providing the best clinical, social, psychological and well-being support without disruption. We want to ensure that all underserved communities get all services necessary to achieve health equity and health care equity. Our population consists primarily of African American and Hispanic Seniors with multiple chronic conditions, social needs, behavioral needs, and well-being needs. We have been successful over the past 30 years helping minority seniors, but we are dedicated to continuing all efforts to ensure that this community is served and represented so they may achieve health.
Intensive Care Management
Our health care professionals have been delivering intensive and high-risk care management over the last 30 years in the community. We understand the multi-dimensional and evolving needs. We deliver the entire spectrum of patient health needs including resources to address social determinants and behavioral needs. We deliver intensive care management services through our home health services and through our virtual delivery of care. Our professionals have been successful through the years reducing unnecessary complications, ED and Inpatient admissions.
Experienced Home Care Team
Our leadership and nursing staff have been delivering home care services for over 30 years. All our nurses have experience in community care management and behavioral health. They not only address clinical needs, but are experienced in LTSS, governmental resources and community resources. We ensure patients have sustainable resources to self-manage at home independently or with assistance. Our registered nurses create a sustainable system of support for the patient so they can remain at home and thrive.
Advanced Virtual Care
Avocare provides all virtual care including telehealth, remote physiological monitoring, and registered nurse clinical call center. Through our telehealth we can assess care management patients as needed as well as delivering behavioral health services including therapy and psychiatry. We have 5G Remote Physiological Monitoring for patients that qualify with appropriate diagnosis and need for monitoring. Our Clinical Call Center is available during and after clinic hours and is staffed with Registered Nurses using evidence-based decision tools.
